ファイル名はそのまま、中身だけが他の画像に挿し変わる事例が発生しています。ファイル名が正しいのでアップロードのミスという可能性はありません。おそらく、ページを作る過程で重複ファイルが発生したタイミングで、何らかのエラーが時々起こるのだと思います。そこらへんに注意してみます。
